PLEASE READ ALL IMPORTANT INFORMATION BELOW AND ATTACHED, BEFORE COMPLETING THE 2025 AUSTRALIAN YOUTH WATER POLO CHAMPIONSHIPS TEAM ENTRIES FORM

Please complete ONE FORM PER CLUB, ONLY (you will be able to add multiple teams in the Gold or Green division in the one form).

Please select the number of 'teams' from the drop down per division you would like to nominate into. Remember, the Gold Division is the top division (1), and Green is the second division (2). 

$395.00 per team deposit IS required upon nomination.

Team Entry registration closes 19 January, 2025... registrations received after this date will incur a 10% late fee of the team entry (example if the Team Entry $3,950 (including GST) the late fee would be an additional $395.00 per team entry).   Remaining fees will be invoiced after the deadline date and are to be paid by 7th March 2025.

Please know any withdrawal of teams after Sunday February 2, 2025, team deposits paid at the time of registration, will not be refunded.

Once the team is added to the MemberPool, you will receive notification providing you with access. 

The person completing this form will be added to each of the team lists once nominated, where it will then be their responsibility to add all coaches, managers, table officials and players along with their cap numbers. 

In the spaces provided, please write an example of how you would like your Teams named.

If there are additional Club Representatives to be added to the Team List, please provide the detail in the space below.
