Reporting Forms

Report a Concern or Complaint
Use this form to report a concern or complaint relating to integrity, conduct or member protection under WPA policy.
Officials Game Report Form
For officials to submit a formal report following a match – including incidents, conduct issues or other observations.
Club Game Report Form
For clubs to submit a post-match report. Required for all sanctioned competitions under WPA rules.
